As the story goes, this Miraculous Medal (also known as the Medal of the Immaculate Conception) was created by Saint Catherine Labouré following 2 visions of the Virgin Mary. In the first encounter in July 1830, Mary told Catherine that God wished to charge her with a mission (no pressure), that she would be contradicted (can't a girl get a break), but not to worry - she had all the grace to do what was necessary (we should be so lucky).
In the second encounter when Mary appeared to Catherine, she was standing inside an oval frame, on a globe. She was wearing many rings of different colors, most of which shone rays of light over the globe. Around the margin of the frame appeared the words "Ô Marie, conçue sans péché, priez pour nous qui avons recours à vous". In English, "O Mary, conceived without sin, pray for us who have recourse to thee."
As Catherine looked on, the frame seemed to rotate. In the frame, there was a large letter M with a cross, a circle of twelve stars, and the Sacred Heart of Jesus and Immaculate Heart of Mary pierced with a sword underneath.
When asked why some of her rings didn't shine light, Mary reportedly said “Those are the graces for which people forget to ask.”
She then asked Catherine to take these images and put them on medallions, saying "all who wear them will receive great graces." - So sayeth Wikipedia
